Hyperammonemic encephalopathy: time course of MRI diffusion changes.
نویسندگان
چکیده
A 55-year-old man with cirrhosis presented with hyperammonemic encephalopathy, subsequently fatal (plasma ammonia level 410 M/L). Three-Tesla MRI initially showed restricted diffusion (figure 1A) in the cerebral cortex, suggesting poor prognosis,1 thalami, and striatum. Apparent diffusion coefficient pseudonormalized 8 days after admission (figure 1B), comparable to the evolution of hypoxic encephalopathy.2 T2 hyperintensity of the gray matter progressively increased (figure 2A). High glutamate plus glutamine without lactate was detected on day 2 in the right thalamus, while N-acetylaspartate progressively declined (figure 2B). These imaging findings illustrate the time course of hyperammonemic encephalopathy.

Valproate-induced encephalopathy: assessment with MR imaging and 1H MR spectroscopy.
The anticonvulsant agent valproate (VPA) may cause hyperammonemic encephalopathy. Magnetic resonance imaging (MRI) and proton MR spectroscopic (MRS) findings in a patient with VPA-induced hyperammonemic encephalopathy are described. MRI showed a metabolic-toxic lesion pattern with bilateral T2-hyperintense lesions in the cerebellar white matter and in the globus pallidus. MR spectroscopic findi...
متن کاملExtensive Cortical Diffusion Restriction in a 50-Year-Old Female with Hyperammonemic Encephalopathy and Status Epilepticus
Comorbid hyperammonemic encephalopathy (HE) and status epilepticus (SE) leading to extensive cortical diffusion restriction (CDR) on MRI have not been previously reported. We describe a patient with HE who subsequently developed provoked SE. Sequential MRIs demonstrated a progressive CDR that involved the entire bilateral supratentorial cortex, thalami, and basal ganglia, resulting in death fro...
متن کاملLate presentation of fatal hyperammonemic encephalopathy after Roux-en-Y gastric bypass.
Worldwide, there have been <25 reported cases of hyperammonemic encephalopathy associated with Roux-en-Y gastric bypass surgery in the absence of cirrhosis. We describe a 42-year-old woman who presented with subacute but progressive neurological decline late in her postoperative course, which deteriorated despite multiple conservative and aggressive measures, including hemodialysis, in an attem...
متن کاملAcute hyperammonemic encephalopathy in adults: imaging findings.
BACKGROUND AND PURPOSE Acute hyperammonemic encephalopathy has significant morbidity and mortality unless promptly treated. We describe the MR imaging findings of acute hyperammonemic encephalopathy, which are not well-recognized in adult patients. MATERIALS AND METHODS We retrospectively reviewed the clinical and imaging data and outcome of consecutive patients with documented hyperammonemic...
متن کاملA Discounted Interaction, Divalproex sodium and Topiramate Induced Hyperammonemic Encephalopathy – A Case Report
An interaction between divalproex sodium and topiramate may induce hyperammonemic encephalopathy, a well-documented, but seemingly overlooked or minimized adverse drug interaction. This case report highlights a 54 year old Caucasian female who presented with sedation, mental status changes, and a history of repeated hospitalizations of similar presentation along with hyperammonemia. The patient...
